Privacy Policy

Effective 23 September 2026.

What this policy covers

This policy explains what happens to information collected through this website. It is written for people browsing the site, whether or not they are patients.

It does not cover your dental records. Information about your care is protected health information, and it is governed by the federal Health Insurance Portability and Accountability Act (HIPAA), the rules the U.S. Department of Health and Human Services issues under it, and the Texas Medical Records Privacy Act. Those rules are set out in our Notice of Privacy Practices, which is at the end of this page and also available at the practice. If you have a question about your records, please read that notice rather than relying on the rest of this page.

We are Smile Design Family Dentistry, 907 Pollard Street, Suite 110B, Dallas, TX 75208.

What we collect

Information you give us

If you fill in the contact form on this site, we receive your name, email address, phone number and whatever you write in the message. That is the only form on the site. We ask you not to include details about a medical or dental condition in it, because a website form is not a secure way to send health information. If you need to tell us something clinical, please call us on (469) 250-4775.

Information collected automatically

When you visit, our website and its host record technical information: the pages you look at, the approximate area you are in based on your IP address, your browser and device type, and the site that referred you. Our security software also records IP addresses in order to block attacks.

What we do not collect here

We do not take payments on this website. We do not ask for insurance details, dates of birth or any clinical information through it. When you click Schedule An Appointment you leave this site for our booking provider, and anything you enter there is collected by them under their own terms.

How we use it

  • To answer you when you get in touch
  • To understand which pages people find useful, so we can improve the site
  • To keep the site online, secure and working
  • To meet our legal and professional obligations

We do not sell your personal information, and we do not share it for targeted advertising. There are no advertising or retargeting trackers on this website.

The services this website uses

Each of these companies handles data under its own privacy policy.

  • Google Analytics and Google Tag Manager. We use these to count visits and see which pages are read. Google Analytics uses cookies to tell a new visit from a returning one. The reports we see are aggregated, and we do not use them to identify anyone. Visitors in the European Economic Area, the United Kingdom and Switzerland are not given analytics cookies unless they agree to them. You can opt out at any time with Google’s Analytics Opt-out Browser Add-on. Google’s policy: policies.google.com/privacy.
  • Google Maps. Our contact page embeds a map so you can find us. Loading that map sends your IP address to Google.
  • Our appointment booking provider. The Schedule An Appointment links take you to a scheduling service hosted elsewhere. Information you enter there is held by that provider, not by this website.
  • Akismet. Contact form submissions are checked for spam by Akismet, operated by Automattic. The message and the sender’s details are sent to its servers for that check.
  • Wordfence. Our security software records IP addresses and request details to identify and block malicious traffic.
  • Our web host. The site is hosted on Kinsta and served through a content delivery network. Both keep standard server logs and may set cookies needed for performance and security.

Cookies

Cookies are small files a website stores on your device. This site uses very few.

  • Necessary cookies. Our website builder sets a session identifier and counts page views within a visit, so that features like the welcome screen behave correctly. Our host and security software may set cookies to keep the site available and safe. These cannot be turned off without breaking the site.
  • Analytics cookies. Google Analytics sets cookies, usually named _ga, to count visits and returning visitors. They are not set for visitors in the European Economic Area, the United Kingdom or Switzerland unless they agree to them.
  • Advertising cookies. None. There is no advertising pixel on this site.

You can delete or block cookies in your browser settings. Doing so will not stop you using the site.

Your choices

Texas has a consumer data privacy law, the Texas Data Privacy and Security Act. It does not apply to us, because the law exempts entities covered by HIPAA (Texas Business and Commerce Code, Section 541.002(b)(3)). Your dental records are protected instead by HIPAA and the Texas Medical Records Privacy Act, which give you the right to see and correct them. Your rights are set out in our Notice of Privacy Practices below.

For the information this website collects, we will do the following whether or not a statute requires it. Contact us and we will:

  • tell you what we hold from your use of this site
  • correct it if it is wrong
  • delete it, unless we have to keep it by law
  • stop sending you anything you did not ask for

We aim to answer within 30 days. If you live in a state with its own privacy law, tell us and we will honour the rights it gives you.

Children

We treat children as patients, and we welcome them. This website, though, is written for parents and adult patients, and it is not directed at children. We do not knowingly collect personal information online from a child under 13, which is the age set by the federal Children’s Online Privacy Protection Act. If you believe a child has sent us information through this site, contact us and we will delete it.

How long we keep things

Contact form messages are kept for as long as we need them to deal with your enquiry and for our records, and no longer than two years unless you become a patient, in which case your records follow the retention rules for dental records in Texas. Google keeps analytics data according to the retention setting on our account, which is never more than 14 months for detailed data. Security and server logs are kept for a short period, usually weeks.

How we protect it

The site runs over an encrypted connection, access to the administration area is restricted and protected by security software, and we limit who can see contact form submissions to the people who need to answer them. No website can promise perfect security, and we will not pretend otherwise.

Links to other sites

This policy covers this website only. Our Facebook and Instagram pages, our booking provider, Google Maps and any other site we link to have their own policies.

Changes

If we change how this site handles information, we will change this page and update the date at the top. Material changes will be described here rather than made quietly.

Your Rights

When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.

Get an electronic or paper copy of your medical record

  • You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. Ask us how to do this.
  • We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee.

Ask us to correct your medical record

  • We will say “yes” to all reasonable requests.

Ask us to limit what we use or share

  • You can ask us not to use or share certain health information for treatment, payment, or our operations. We are not required to agree to your request, and we may say “no,” for example, if it could affect your care. If we agree to your request, we may still share this information in the event that you need emergency treatment.
  • If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.

We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.

Choose someone to act for you

  • If someone has authority to act as your personal representative, such as if someone has your medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
  • We will make sure the person has this authority and can act for you before we take any action.

File a complaint if you feel your rights are violated

  • You can complain if you feel we have violated your rights by contacting us using the privacy contact details at the top of this notice.
  • You can file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting www.hhs.gov/hipaa/filing-a-complaint.
  • We will not retaliate against you for filing a complaint.

Your Choices

For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions.

In these cases, you have both the right and choice to tell us to:

  • Share information with your family, close friends, or others involved in your care or payment for your care
  • Share information in a disaster relief situation
  • Include your information in a hospital directory If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest. We may also share your information when needed to lessen a serious and imminent threat to health or safety.

In these cases we never share your information unless you give us written permission:

  • Marketing purposes
  • Sale of your information
  • Most sharing of psychotherapy notes

In the case of fundraising:

  • We may contact you for fundraising efforts, but you can tell us not to contact you again.

If we have your substance use disorder patient records, subject to 42 CFR part 2, we will give you clear and obvious notice in advance and a choice about whether to receive fundraising communications that use your Part 2 information.
